We support health organisations and local councils to develop local partnerships to coordinate services around the whole needs of each person to get the care and treatment they need, in the right place, at the right time.
Our role is to support the set-up and implementation of integrated care system in collaboration with organisations, local councils and others to take collective responsibility for managing resources and improving the health of the population they serve.
Working through organisations, we facilitate and support to create full participation of local stakeholders and their organisations, service users and carers in the design and delivery of integrated care in their community.
We take systematic approaches to understanding population health needs through efficient, standardised processing, collation and analysis of data from various parts of the health and care system to aid with successful implementation of integrated care.
